Change of address and name
1. Request for personal particulars change / online service account registration ([R8])
2. OnePlatform's Personal Details Update Form
3. CS eform
4. For permanent address update, certified address proof is required to submit. (Updated: 17 Jan 2014)
Client needs to fulfill both principal and OWM address proof requirement, for details, please refer to:

5. For correspondence address update, NO NEED to submit certified address proof.
6. For name update, the certified true copy of name change deed poll and the new ID (passport) showing the new name are required. If the change of name is due to marriage, a certified true copy of the marriage certificate can be submitted in place of the name change deed.
