Important Notice
The ownership of Cigna Worldwide Life Insurance Company Limited (“Cigna Life”) was transferred to the Chubb group in July 2022 and Cigna Life had been renamed to “Chubb Life Insurance Hong Kong Limited”. The policies originally under Cigna Life have already been transferred to one of the following companies: (1) Chubb Life Insurance Hong Kong Limited or (2) Cigna Worldwide General Insurance Company Limited. Meanwhile, please note that Chubb Life Insurance Hong Kong Limited and Chubb Life Insurance Company Limited are two different entities currently. To avoid using incorrect provider’s form and information, it is suggested you send Service Log to check the latest policy information. (Updated: 12 Apr 2024)
1. Most documents should be submitted to SOD within 30 calendar days after sign date of client.
2. OnePlatform's office address is NOT accepted as correspondence address of client.

4. It is not required to submit the certified address proof if the client changes the correspondence address.
5. Address proof is not required unless it is required by Cigna, or OnePlatform has to conduct further verification with the client’s identity. (Updated: 24 Oct 2017)
