Assignment - Prudential

Important Notice

  1. Prudential forms should be submitted to SOD within 26 calendar days after sign date of client.
  2. Person aged below 18 cannot be a Policy Owner
  3. Financial Consultant cannot be the Assignee except whose relationship with Life Assured is spouse, parent, children or sibling. For relationship of Sibling, the supporting document is required
  4. Assignment is not allowed if either the Assignor or the Assignee (if he / she is an existing client) is bankrupted
  5. If the Assignee is not an existing client, the client record has to be created prior to the input at the Policy.  Customer Due Diligence (CDD) requirement is required
  6. Country Code, Area Code and Country Name of Assignee are required

Checklist

1.  Transfer of Policy Ownership Form ([T2])

2.  CS eform [from the assignee]

3.   Copy(ies) of Identity document(s) (from new owner)

4.  Address Proof (from the assignee)

Client needs to fulfill both principal and OWM address proof requirement.

5.   Client Agreement (CA form) (from the assignee)

6.   Change of Payment Mode and Direct Debit Authorization Form - Bank Account ([C4]) (From the assignee) (If applicable)

7.   Cheque payment of 2 months premiums (If client want to change the policy payer)

Since DDA needs 2-3 months to set up

8.  Relationship Proof (Existing & new policyowners is Parent, Spouse or Children)

Additional Requirement​

  • Part 6 in "Transfer of Policy Ownership Form" is only applicable to Primary and Secondary Beneficiary Appointment to align with execution of policy provision.  If Tertiary Beneficiary Appointment is required, please complete Supplemenary Information Form LAXXD017E ([S1]) and submit with the "Transfer of Policy Ownership Form".
  • For the Request for Transfer Policy Ownership and appoint a Minor Beneficiary and appoint of Trustee for Minor Beneficiary, you must submit a request by using the revised version of "Transfer of Policy Ownership Form" (Version number: 09/16). The final decision on the validity of this form remains with Prudential.

Mainlander Requirement

 (Holder of either PRC Passport, PRC ID or PRC Travel Permit for HKSAR & MSAR but do not hold HK Identity Card.)

Starting form 1 September 2016, new policyowner must sign "Important Facts Statement for Mainland Policyholder ("IFS-MP")" ([I2]) (IFS-MP).

Under the below conditions, new policyowner MUST sign the IFS-MP at Hong Kong and go through Mainland Sales Verification at our Integrated Service Center.

  • Within the first year from the Date of Policy or Commencing Date (whichever is earlier); AND
  • New Policyowner is Mainlander: AND
  • Existing and New policyowner are not direct family members (i.e. Parent, Spouse or Child)
