Update Personal Details - Blue (Ex-Aviva)

Important Notice

1.   All the form details must be filled in ENGLISH.

2.  OnePlatform's office address is NOT accepted as correspondence address of client.

If consultant is the same as the client then OnePlatform office address is acceptable as correspondence address

3.  The submission of document must be within 1 month from the sign date

Checklist

1.  Request for Personal Details Change Form ([R5])

2. Self-Certification Form (Individual) [S7] or Self-Certification Form (Entity) [S8] or Self-Certification Form (Controlling Person)  [S9]

**Remarks: 1) Self-Certification Form is required if client change to overseas (non Hong Kong) mailing address, residential address, and telephone number.

2) Self-Certification Form is not required if client submit the form to update the client signature, email address and re-set Blue Online Service. (Updated: 11 Jan 2021)

5.  Certified ID copy

The client should also submit a copy of his / her most updated identification document to Blue together with their policy service request unless the same has been submitted before.
