4. All the missing premium
5. Direct Debit Authorisation ([D1]) (if applicable)
- If the DDA is invalid, please submit 2 months advance premium for DDA set up. Depends on previous payment status/ DDA status.
6. W-9 or W-8BEN (if applicable), please refer to Important Notice item 2 for details.
