Checklist

1. CS eform

2. Certified copy of policy owner's and insured's HKID (for HK residents) or Passport (for Non-HK residents)

3. Request for Change In Policy Form (POS069) ([R1])

4. All the missing premium

5. Direct Debit Authorisation ([D1]) (if applicable)

  • If the DDA is invalid, please submit 2 months advance premium for DDA set up. Depends on previous payment status/ DDA status.

6. W-9 or W-8BEN (if applicable), please refer to Important Notice item 2 for details.
