2. Applications forms are valid for 3 months from signing, after which a Declaration of Continued Good Health will be required. The underwriter may require additional information.

5. All samples provided in OnePortal are for REFERENCE only, consultant must fill in all forms with client’s latest information provided. The reference products recommended should be on OnePlatform’s shelf by the time you make the recommendation. - P.19 Witness to signature of proposed insured

- P.25 of Report by Insurance Intermediary, Signature of Authorised Producer
- For the answer in Section 7 is "yes", please complete the Supplementary Questionnaire.
- For the answer in Section 7 Q7.3 is "yes", please complete Application of Supplement Avocation & Sport Questionnaire ([A2])
- For Beneficiary, limit to Immediate family members only. Client can name "Own Estate" at application stage
