Important Notice:

2. Application Form should be delivered to FWD within 30 days (7 days for non-HKID holders) after the client signs the form, otherwise new application form should be signed. (Updated: 21 Jan 2021)
3. DDA form have to been submitted with the application document.
4. Except the application form, other printed copies of forms are accepted.
5. Please note that the Altitude II and Vintage do not share the same fund list.

- Please write your FWD individual code (last 5 digits of TR’s Portal ID) on the application form.

- If policy owner is different from life insured, they should have insurable interest

- Exchange Rate of USD for Premium should refer to fwd.com.hk updated by each Wed
- For the AFYP > HKD 80,000, simplified underwriting is required. Please complete Question 19 on the application form. If one of the answers of Question 19 is "YES", please complete the section G on the application form.
- Every individual customer must declare whether he/she is a tax resident of any jurisdiction(s) other than Hong Kong and the U.S. (Declaration relating to Automatic Exchange of Financial Account Information) If a client is working in Hong Kong but residing in Mainland/Macau and declares “No” in this section, he/she must declare by Addendum that 1) he/she is working in Hong Kong and only subject to Hong Kong taxation; and 2) despite the fact that he/she is residing in Mainland/Macau, he/she is NOT a tax resident of Mainland/Macau.
2. If the customer’s answer is “Yes” or “Not yet decided”, the licensed insurance intermediary must explain to the customer and help him / her complete the “Important Facts Statement - Policy Replacement [I3] ".(Updated: 12 Mar 2021)
- For Direct Deposits, Bank Draft Cashier Order, Telegraph Transfer, PPS need to submit Premium Payment Declaration ([P5])

5. Financial Needs and Investor Profile Analysis Form ([F1]) – RPQ is required, please submit p.7 - 10. As OnePlatform FNA is a must form for new application and it is also accepted by FWD, so it’s not a must to submit FWD FNA form at the same time (Updated: 13 Apr 2021)

- In the case of CRS indicia is found on application documents, alternatively client can cure the indicia by providing document proof instead of completing the form.

- For the purpose of sales quality assurance and enhancement of customer engagement, FWD have kicked off welcome calls exercise for different channels from 27 Oct 2020. Policies will be selected in random basis and FWD will make a call to the selected client. Please refer to the Outbound Call Scripts for reference (Updated: 5 Nov 2020).
