Client can opt-out through the below methods:
1. CS eform
2. Client may submit a written request saying that they would like to opt-out and state clearly the full name,
policy number and client’s signature.
Or
1. CS eform
2. Supplemenary Information Form LAXXD017E ([S1]) fill up the form saying that client would like to opt-out and client’s signature.
