2. Self-Certification Form for Individual ([S4]) (If applicable)
- An individual client will need to complete the new "Self-Certification Form for Individual if
1) he/she declares "Yes" in the Declaration relating to Automatic Exchange of Financial Account Information on application form; or 2) any of the CRS indicia listed above is found on application documents
3. Certified true copy of identity document of Policy Owner if it has not been submitted. For non-Hong Kong/Macau permanent resident, nationality proof/travel documents copy (e.g. passport copy) is also required.
4. Original Policy Document (if any)
5. CS eform
6. Address proof is not required unless it is required by FWD, or OnePlatform has to conduct further verification with the client’s identity. (Updated: 4 Dec 2017)
