Important Notice

1. The original death certificate that will be certified as a true copy by Transamerica’s office staff.

2. The original document will be immediately returned to the beneficiary / OnePlatform.

Checklist

Initiating a death claim request

When being aware of the Insured’s/Survivor Insured’s (as applicable) death, please contact Transamerica via telephone or email or inform OnePlatform and provide the following details to help Transamerica expedite the initial claim handling:

  • Policy number
  • Insured’s name
  • Beneficiary’s name and address
  • Date, place and nature of death, if known

Then Transamerica will issue requirement letter to policy beneficiary accordingly.

Documents required for processing a death claim

1. Claim Form ([C7]) – completed and signed by the claimant/Beneficiary. All Beneficiaries must complete a separate Claim Form and sign their full names and provide ID (passport) copy. If the primary Beneficiary is deceased, a death certificate confirming this is also required.

2. Original Death Certificate of Insured.

3. Relationship proof between Insured/Survivor Insured and Beneficiary(ies).

4. Original Policy contract.

Remarks:

1. Depending on the Beneficiary (Estate, Minor, Trust, Corporation or an Assigned Policy) additional documentation may be required.

2. If the death occurred outside the country of residence of the Insured/Survivor Insured, an Overseas Death Questionnaire Form will be required. Copies of police reports, coroner’s inquest, newspaper clippings related to the death (if applicable) may be required for processing the claim.

Claims settlements

It generally takes 5 working days from the date of notification of death for Transamerica to complete their initial claim handling.

They will correspond directly with the Beneficiary and copy the authorized distributor.

Once all claim requirements are received at Transamerica branch office in good order and the claim is incontestable and non-suspicious, they will generally take 10 working days to complete the processing and settlement.

The claim will be settled in USD by telegraphic transfer. For Hong Kong issued Policies and claims made, Transamerica may offer payment in Hong Kong dollar by adopting a floating exchange rate specify by them. Further, they may prepare payment by cheque if the amount is less than USD500,000.
