• Third Party Payment Declaration Form (please kindly refer to Form [T1])
• Identification document of 3^rd party payer (Certified true copy)
• Payment proof (Certified true copy) and/or new recurring payment method form
• Address proof is not required unless it is required by Heng An Standard Life, or OnePlatform has to conduct further verification with the client’s identity.  (Updated: 2 Nov 2018)
• * An additional Source of Wealth document may be required after assessment

Notes:

• Acceptable for payees resident in approved countries as per new business acceptance countries
• The third party payer is subject to the same requirements for source of fund and source of wealth as the policy owners
