Change of Payment Method - Cigna

Important

The ownership of Cigna Worldwide Life Insurance Company Limited (“Cigna Life”) was transferred to the Chubb group in July 2022 and Cigna Life had been renamed to “Chubb Life Insurance Hong Kong Limited”. The policies originally under Cigna Life have already been transferred to one of the following companies: (1) Chubb Life Insurance Hong Kong Limited or (2) Cigna Worldwide General Insurance Company Limited. Meanwhile, please note that Chubb Life Insurance Hong Kong Limited and Chubb Life Insurance Company Limited are two different entities currently. To avoid using incorrect provider’s form and information, it is suggested you send Service Log to check the latest policy information. (Updated: 12 Apr 2024)

  • Cigna takes 6-8 weeks’ time to process this authorisation, so the payment of two months’ and all outstanding premium is requested to be sent along with this Authorisation Form.
  • At least 5 working days’ written notice in advance is required for termination of this payment instruction.

  • All outstanding premium is requested to be sent along with this Authorisation Form.
  • This facility is applied to Policyholder, Beneficiary or Person Insured/Proposed Person Insured only.
  • This payment method is for regular premium and selected products only.
  • Non-monthly premium will be debited on the same month of the premium due date and on the Payment Date chosen below. In case of unsuccessful transaction, premium will be debited again next month.
  • At least 5 working days’ written notice in advance is required for termination of this payment instruction.
