Important Notice (Updated: 14 Aug 2012)
1. Kindly send service log of reinstatement enquiry to OnePlatform CS before submission.
2. The reinstatement request must be submitted within 12 months from the last Paid-to-Date.
3. Reinstatement is applicable for policy ‘lapse/surrender’ from non-payment of premium only. Not applicable for policy that was requested to surrender by the client.
4. Please note that starting from 1st April, 2012, all initial premium/ contribution payment must be made by the applicant/ policy owner. Any third party payments for the initial premium/ contribution received by Heng An Standard Life on or after 1st April, 2012 will be rejected.
5. New recurring payment method is required for montly cases.
6. MVA may be required. Will be calculated after receiving client's original written request.
7. Client need to settle all premium due from the last paid-to-date until now.
8. Return of the cheque issued by Heng An Standard Life on surrender value (if any).
9. HKID/passport of policy owner if not previously submitted.

11. Per OCI require that any new business applications, top-up applications, reinstatement requests from Mainland Chinese Policyholders and assignments (Including collateral assignment) to a Mainland Resident Identity Cardholders, should sign an additional Important Facts Statement for Mainland Policyholders (IFS-MP) form, effective from 1 Sept 2016. Please note that the IFS-MP form must be signed and submitted together with the application/top up/reinstatement request/assignment forms.
Checklist
For reinstatement from Lapsed policy
1. CS eform
2. Policy Alteration Form for Investment Plan - Form A (please kindly refer to Form [P1]) for reinstatement signed by the policy owner.Please provide all of requested personal information. Any incomplete personal information may result in a delay or rejection in processing your request. (Updated: 5 Jun 2024)
3. Payment of all overdue contributions and market value adjustment (MVA) to cover any investment loss resulting from realizing and re-subscription of investment contribution, if any. MVA (if any) will be advised at the time of all reinstatement documents have been received by Heng An Standard Life.
4. New Direct Debit Authorization / Recurring Credit Card Instruction / Bank Standing Order for monthly cases. Heng An Standard Life does not accept any subsequent premium/ contribution payments made by a third party.
5. If reinstatement request was being closed from no submission of outstanding requirements at pending status by deadline, a new set of reinstatement documents and payments (including re-calculation of MVA) is required to be re-submitted. No re-open of closed reinstatement request.
6. Address proof is not required unless it is required by Heng An Standard Life, or OnePlatform has to conduct further verification with the client’s identity. (Updated: 2 Nov 2018)
7. Submit HK ID card / passport document copy of the Policy Owner / Beneficial Owner / Assignee if it has not been submitted.
8. Important Facts Statement for Mainland Policyholders (IFS-MP) form - (please kindly refer to Form [I7]) - Required for the policyholder is a holder of Resident Identity Card (PRC).
9. Complete the Source of Wealth Verification in Part 1 and sign the Declaration and Signature in Part 3 in Change in Contribution Form (Please refer to Form [C3])
10. Health Statement Declaration Form (Please refer to Form [H1]) if there is any rider(s) attached.
For reinstatement from Contribution Holiday (Updated: 8 Feb, 2013)
1. Policy Alteration Form - Form B (please kindly refer to Form [P2])
Please tick the box "Resume paying contribution" with effective date under Section 1 "Stop / Resume Paying Contribution"
