Important Notice:

1. All forms must arrive Blue office within 30 calendar days from application form sign date, please submit at least two working days before the deadline to OWM.

2. All samples provided are for REFERENCE only, TR must fill in all forms with client’s latest information provided. The reference products recommended should be on OWM's shelf by the time you make the recommendation. No same provider’s products should be recommended unless there are no other suitable products from other providers.
