4. Risk Profile Questionnaire [Sample] (Updated: 07 Feb 2025)
- Commenced on 1 Oct 2024, all ILAS new business (including top up) must submit with OWM RPQ.
- OWM RPQ must submitted to SOD within 30 calendar days after signed by client.
- Risk tolerance of 1 level up/down between OWM RPQ & Provider RPQ is acceptable.

7. Transfer Servicing Rights

7.1 External Transfer in
Remark: Please take and submit the above form for external transfer-in from CFS to OWM. Clients who have no in-force policies in CFS on or before 4 Mar 2022 are not eligible for simplified transfer-in and will be required to submit a full set of necessary documents for successful transfer-in. You may want to refer to the above link for Important Notes before proceeding submission.

- For YF Life TR Record Update:
- Request for Policy Change Form (A01B) (please kindly refer to Form [R3]) must be submitted as required by YF Life.
- Client must sign on each page of the form (even blank page).
- Please ensure the new consultant has existing YF Life TR code
- If the client and the consultant were one and the same person, he/she must be the servicing consultant.
- As LY455 code is not applicable for PRC case, therefore, new TR must obtain PRC license for PRC case. (Updated: 15 Jun 2018)

9. MPF (Updated: 10 Apr 2024)
- Rules of Submission
- MPF submissions are only accepted through New Business Submission Dropbox (NB Dropbox)
- According to OWM MPF Selling Process Procedure, MPF forms and documentation should be submitted to SOD within 7 working days after signup. SOD reserves the rights to reject submission exceeded such timeframe.
- MPF Suitability Assessment (MPFSA) is necessary for each asset transfer and fund transfer.
- If the asset transfer and fund transfer forms are supplementary of new business submission, MPFSA is not required but consultant must provide Trustee email or service log to support the supplementary is requested by Trustee.
