Assignment - Blue (Ex-Aviva)

Important Notice

1.  All the form details must be filled in ENGLISH.

2.  Blue DO NOT accept consultant as assignee unless the consultant is a spouse, immediate family members of the policyholder. Immediately family means Parent and Children.

3.  Blue accept assignment from client to his/her friend provided that 3rd party witnessed on the Deed Assignment Request Form.

4.  Even the policy owner can be changed; the original life assured cannot change after the policy start.  Therefore, the life assured will not be changed even the assignment is completed.

5.  Accept assignment without insurable interest between LIfe Assured and Assignee.

6. Single applicant can only assign his/her plan to a single person.

7. The submission of document must be within 1 month from the sign date.  Existing auto pay mandate would be cancelled after assignment completed.

Checklist 

1. CS eform [from the assignee]

2.  Change of Policy Ownership ([C3])

3.  Certified Address Proof [from assignee]

Client needs to fulfill both principal and OWM address proof requirement AND Blue- Address Proof Requirement.  (within 3 months)

4.  Certified HKID copy [both assignor and assignee)

5.  Client Agreement (CA form) [from assignee]

6.  Risk Profile Questionnaire ([R9])

7. Self-Certification Form (Individual) [S7] or

Self-Certification Form (Entity) [S8] or

Self-Certification Form (Controlling Person)  [S9]

For Non-HK assignee (Additional Document)

1.  Certified true copy of the passport / visa

  • To show the entry / exist date into / leave Hong Kong with Hong Kong entry stamp.The pages of clients signature and photograph in the passport are also required. Mainland visitors should be submitted with WHOLE passport copy.

2.  Certified true copy of the proof of traveling, such as boarding pass, air ticket, ship / train ticket, is highly recommended but not mandatory.
