Important Note:
If the policy holder needs to change the percentage of beneficiaries or reallocated beneficiaries after the policy has been issued, please note the following:
- Policy owner need to submit ‘Policy Donation and Beneficiary Appointment Form, if a designated beneficiary is a non-immediate family member of the insured person, certified copy of identity document is also required.
- If there are more than one beneficiary, please specify the distribution ratio on the application form. If a clear allocation ratio is not provided, the Policy Services Department will issue a follow-up letter to the policy owner.
- This application only accept "primary beneficiary" and "secondary beneficiary". China Life does not accept "third beneficiary". The designated second beneficiary shall enter into force after the death of all primary beneficiaries.
- Please note that the total percentage of the beneficiaries of the same category (i.e. primary or secondary) must be 100%.
