OnePlatform Internal Document:
1. CS eForm [from the assignee]
2. Client Agreement (CA form) [from the assignee]
Principal Document:
1. Change of Policyholder Form (Individual) ([C2]).
2. Certified copy of new policyholder’s HK Permanent ID card and /or certified copy of valid passport (if holds foreign nationality) is required.
3. Copy of the new policyholder’s travel document showing a relevant Hong Kong entry stamp must be provided (if applicable).
3. Important Facts Statement for Mainland Policyholders (IFS-MP) Form ([I7]) If residential country is China or nationality is Chinese and without HKID (Mainland China does not include HKSAR).
