2. Certified true copy of identity document and address proof of Policy Owner. For non-Hong Kong/Macau permanent resident, nationality proof/travel documents copy (e.g. passport copy) is also required.

Lost of Policy
If the client has lost the policy schedule, he/she will need to complete the Lost Policy Schedule Questionnaire ([L1]) and send the original to us with the surrender request form. Upon our acceptance of the questionnaire, we will then send a Form of Indemnity ([F2]) for the client to complete. Two parts process and must be completed separately.
