With immediate effect from 3 April 2019, Fubon Bank will not accept cash / cheques / bank drafts deposited to Fubon Life’s bank accounts for premium payment. Customer may bank-in payment to Fubon Life’s Citibank accounts instead. Telegraphic transfer to Fubon Bank accounts is still accepted by Fubon Life. (Updated: 4 Apr 2019)

Notes:
(1) Customer needs to send in proof of payment (e.g. pay-in slip) showing the source account no. and full name of account holder, together with policy no. and name of policyowner / Insured person to Fubon Life (if renewal premium, attention of Policy Administration Department) by mail or email within 7 days of payment. All bank charges will be borne by the customer.
- For bank direct transfer, bank draft or TT payment, bank proof showing the identity of payor and payment source is required:
○ Bank direct transfer: bank proof of pay-in slip showing the source account number and full name of account holder is required. If incomplete information of account number or account holder, customer need to provide other supplementary bank proof (e.g. bank statement).
○ TT payment: TT covering letter showing the source account number and full name of account holder is required.
○ Bank Draft: bank proof showing the name & identity of payor is required.
- Customer should request the bank to quote the application no. / policy no., name of proposed policy owner in the reference section.
- If Fubon Life is not notified of any payment and is not able to identify the source of payment within three weeks from the date of receipt, the payment will be refunded to the remitter and subject to all bank charges deducted in the course of refund process.
- Local cheque or bank draft through Citibank payable to "Fubon Life Insurance (Hong Kong) Company Limited"(in HKD or policy currency) (Cheque date MUST be on or after application sign date.) (Updated: 27 Feb 2019)
(2) Cash must bank-in through Citibank (all branches), payor may at the request of designated bank needs to provide identification documents and to sign the pay-in slip and send related pay-in slip, completed and signed Initial Premium Payment Declaration Form to Fubon Life within 7 days of upon payment.
(3) Visa Card or MasterCard Credit Card payment by credit card machines at Fubon Life Office. Administration charge 1.4% on premium paid amount will be incurred and born by payor if the VISA / Master Card payment is not acceptable for the product, and "Request for Payment by Visa Card / MasterCard Credit Card Form" is required.
(4) Accept Visa Card or MasterCard Credit Card issued by banks in Hong Kong only. "Request for Payment by Visa Card / MasterCard Credit Card Form" and "Credit Card Payment Authorization Form" are required. Administration charge 1.4% on premium paid amount will be incurred and born by payor if the VISA / Master Card payment is not acceptable for the product. Points to note for Initial Payment:
- Premium payment should be made by Proposed Policy Owner or Proposed Insured.
- One modal premium is required for semi-annually and annually paid application.
- Payment of 2 months’ premium in advance for monthly mode policies involving the set-up of Direct Debit Authorization ("DDA")
- Initial payment can be paid in either USD or HKD under the following circumstances:

- "Request for Payment by Visa Card / MasterCard Credit Card Form ([R3])" is required for Visa Card or MasterCard Credit Card payment by credit card machines at Fubon Life Office (i.e. Card Present).
- "Request for Payment by Visa Card / MasterCard Credit Card Form ([R3])" is required, and "Credit Card Payment Authorization Form ([C1)]" are required for Visa Card or MasterCard Credit Card payment by card not present.

- Card payments must be made in HKD.
- If policy currency is USD, the card payment will be paid in HKD. The current declared exchange rate is 7.8 and is subject to review and change from time to time by Fubon Life.
- Cardholder should be the Proposed Policyowner or Proposed Insured.
- It is not allowed to separate the premium payment into different transactions by using same card or different cards, unless due to the credit limit of the card.
- Card should bear with cardholder’s signature. Insurance intermediary should request cardholder to sign the card if the signature field is blank.
- Only accept VISA and MasterCard cards. UnionPay, American Express and Diners Club cards are not accepted.
