1. Fill in Request for Change of Policy Value Arrangement Form (A15B) ([R2])

1) If any content on the form is being amended/revised, the proposed policy owner is required to counter-sign close to the revised part as confirmation;
2) If there is more than one type of handwriting on the form (such as part of handwritten content and part of computer-printed content), the proposed policy owner is required to sign on the lower right corner of each page of the form as confirmation.
2. Declaration of Individual Tax Residence (POS-G03CRS) Chi Version / Eng Version（[D4]）- If applicable and subject to principal
3. Certified identity document copy (if a copy of valid identity document has not been provided before)
(a) 香港永久性居民
- 香港永久性居民身份證
(b) 香港居民（非永久性居民）
- 有效旅遊證件；或
- 香港居民身份證及載有個人照片並可證實其國籍之有效文件（如護照、國家駕駛執照等）
(c) 非香港居民之人士
- 有效旅遊證件；或
- 載有個人照片的身分證

Flexi-Annuity Investment Plan (Annuity Option) (Updated: 7 Dec, 2012)
行使年金時必須符合下列要求：
1. 受保人於保單週年日已年屆55歲；及
2. 保單已生效10年；及
3. 現時保單的現金價值已達最低金額，即美元10,000／港幣80,000／澳門幣80,000
當保單已達到上述要求（1）及要求（2）時，YF Life 會於保單狀況年報（Annual Statement）
內提示客戶可開始行使年金權益。
現行之年金權益選擇包括：(Please refer to attachment for details)
- 定額終身年金
- 定額終身年金 - 10 年保證期
- 定額終身年金 - 15 年保證期
- 定額終身年金 - 聯合年金領取人。
辦理行使年金申請之行政程序
客戶如欲保單開始發放年金，可遞交 Request for Change of Policy Value Arrangement Form (A15B) ([R2]) 表格或書面通知予YF Life。請注意，如客戶欲選擇
「定額終身年金 – 聯合年金領取人」，則必須於(A15)表格或書面通知中提供共同年金領取人之
出生日期。
當YF Life收到有關要求後將發出年金權益說明書（Illustration Document for Annuity Option）
予保單持有人。
如客戶沒有提供共同年金領取人之出生日期，則上述說明書不會列載有關年金權益選擇之金額供
客戶參考。
客戶必須詳細閱讀年金權益說明書內之條款及條件，當決定選擇行使年金權益時，請填妥有
關之說明書並寄予本公司辦理有關手續。 各顧問請提醒客戶，一經選擇年金權益並為YF Life
批核，一切已批核之年金安排將不能更改。
YF Life 收妥文件並批核有關申請後，會發信予保單持有人確認年金權益及年金生效日。
