Important Notes

1. Form should be submitted within 26 calendar days to OnePlatform​​​​​​​.

2.  Reinstatement period is subject to product features. In general, reinstatement only allow within 2 years from last Next Due Day

3.  Reinstatement is NOT allowed if the policy was terminated / surrendered by the client

4.  Reinstatement Effective Date is the Reinstatement Approval Date

5.  The Provision Clause of Incontestability, Suicide and Waiting Period will be re-calculated starting from Reinstatement Approval Date

Simplified Reinstatement is applicable to traditional life policy.  The guidelines are as follows:

Insured Age at time of Lapsed Eligibility Simplified Reinstatement Procedures
55 or below
  • Policy has been lapsed within 1 year
  • The lapsed policy with underwriting decision of "Standard to +100% or +100EM (Table D)" with or without exclusion
  • No Claim History under the benefit of Critical Illness/ Waiver of Premium
  • Reinstatement with Re-dating is NOT allowed

Otherwise, policy reinstatement will be subject to existing guideline and the simplified procedures cannot be applied.

  • Underwriting is not required
  • Request Form is not required
  • Only pay back the overdue premium with interest (if necessary)
  • All other administrative rules remain unchanged
56 & above
  • Policy has been lapsed within 6 months
  • The lapsed policy with underwriting decision of "Standard to +100% or +100EM (Table D)" with or without exclusion
  • No Claim History under the benefit or Critical Illness/ Waiver of Premium
  • Reinstatement with Re-dating is NOT allowed

Otherwise, policy reinstatement will be subject to existing guideline and the simplified procedures cannot be applied.

For the policy which does NOT fulfill the Simplified Reinstatement requirements, the required documents for Reinstatement are listed as below:

DOCUMENT / PAYMENT REQUIRED

1.   Change Form (with Health Questionnaire) ([C6])

2.   Overdue premium*

3.   Overdue interest (if the initial overdue premium is overdue for more than 90 days)*

4.   Direct Debit Authorization ([D3]) / Credit Card Direct Debit Authorization Form ([C5])

* (P.S. If payment mode is monthly mode, it MUST be submitted)

* Note : Additional requirements are needed if payment, bank account or credit card account is under a third party.

  • Further requirement and final decision are subject to underwriting assessment
  • Medical costs, if any, will be borne by client
