1. Certified copy of Local Probate or Will

2. Certified copy of the Death Certificate

3. Return of original policy documents OR Never Received / Lost Policy Declaration ([N1]) completed by beneficiary/claimant (s)

4. Certified copy of photo identification / Passport of beneficiary/claimant (s) (standard certification wordings attached)

5. Certified copy of residential address proof of beneficiary/claimant (s) (standard certification wordings attached)

6. Claims for Death Benefit ([C5]) with the full bank details completed by beneficiary/claimant (s)

7. International Tax Compliance (ITC) Form – Individuals ([I5]) completed by beneficiary/claimant (s)

Remark:

If there is no beneficiary nominated AND when the claim is over GBP 10,000 then Utmost are pleased to receive sight of the Guernsey Grant of Probate which will identify the executor of what is essentially the deceased's Guernsey Estate as this policy is issued by a Guernsey Registered company and is governed by Guernsey Law. Probate in Guernsey can be obtained via a local advocate or alternatively from the Guernsey Ecclesiastical Court. However, For Hong Kong and Singapore residents, a Guernsey Grant of Probate is only required if the claim is over GBP 25,000 or currency equivalent. For residents in the remainder of the Far East (outside of Hong Kong), Utmost will require a probate for a claim that is over GBP 10,000 or currency equivalent.
